Eligibility:    Adults with severe and persistent mental illness and/or mental retardation.

Services Provided:    Case managers coordinate the needs of consumers by linking them to services, both within the Center and the community.

Service Area:    Taylor, Jones, Callahan, and Shackelford Counties
